I. Project Overview

The clean operating rooms of the surgical ward building of Jiangsu Provincial People's Hospital are located on the third floor, with a floor area of 2000m² and a story height of 5m. There are 23 clean operating rooms in total (including 12 radiation-proof operating rooms), among which 4 are Class I operating rooms, 5 are Class II operating rooms, and 14 are Class III operating rooms. The clean corridors and the auxiliary functional rooms within the clean corridors are all Class 100000. The equipment floor is located on the floor above the operating rooms on the third floor, with a story height of 3.2m.

II. Plan Layout

The purification project of the clean operating department of Jiangsu Provincial People's Hospital consists of clean operating rooms and the corresponding auxiliary functional rooms. The internal plan layout and corridor arrangement of the clean operating department conform to the principles of short and direct functional flow and clear separation of clean and soiled areas.

This scheme adopts a double-corridor arrangement: the outer corridor is the soiled corridor, where post-operative waste is collected in the waste room for disposal, or transported out of the clean operating department via the soiled elevator; the inner corridor is the clean corridor, through which medical staff, patients, and clean items enter each operating room. The clean–soiled diversion is clear, eliminating cross-infection.

Medical staff change shoes and clothes in the shoe-changing and changing rooms on the second floor, and then enter the clean operating department on the third floor via the dedicated staff staircase. The four Class I operating rooms are arranged at the far end of the clean corridor and separated off by doors, ensuring the high cleanliness of this area. Scrub sinks are distributed throughout the various areas of the operating department for the convenience of medical staff. After recovery from surgery, patients leave the operating department through the door of the recovery room on the side near the waste cart storage room, without having to pass through the clean area again. Sterile supplies are transported from the Central Sterile Supply Department to the sterile room via the clean elevator for use by the clean operating department.

The building of this project is rectangular in shape, and the operating department forms the main circulation route along the long direction of the rectangle, making the layout of the entire operating department fairly orderly, with the routes into each operating room being very short and convenient.

III. Air-Conditioning and Purification System

The air-conditioning system project for the purification operating rooms of the surgical ward building of Jiangsu Provincial People's Hospital adopts a total of 19 Chaowei-type medical purification air handling units, in which each Class I and Class II operating room is equipped with one dedicated unit, each unit is shared by two or three Class III operating rooms, and two units are provided for the clean corridors and clean auxiliary rooms according to zoning.

The air-conditioning system adopts the primary return air mode. Two fresh air handling units are installed in the system to centrally dehumidify, cool, heat, and filter all the fresh air of the system. Through the electric actuators connected to each constant air volume device, regulation can be performed both during normal use of the operating rooms and during on-duty periods, supplying respectively the fresh air for normal use and the fresh air for maintaining positive indoor pressure during on-duty periods.

The functional sections of the purification air handling units for the operating rooms are configured as follows: fan section, air-equalizing and initial/medium-efficiency filtration section, and surface cooling, heating, humidification, and air outlet section.

A dedicated machine room is set up on the floor above the operating floor, used for the installation of the air handling units for the operating rooms and other equipment. Setting up the dedicated machine room above the operating rooms makes the system ductwork shorter, with low resistance and energy-saving operation, and the maintenance of the entire system is also more convenient.

The cooling and heating sources of the system are supplied via a two-pipe mechanical circulation system, and electrode humidifiers are used for humidification.

IV. Indoor Air Distribution

The clean operating rooms adopt a top-supply, bottom-return airflow pattern, in which the Class I operating rooms adopt vertical unidirectional (laminar) flow, while the Class II and Class III operating rooms adopt turbulent flow according to their air change volumes. The system provides a supply air plenum above the operating table in each operating room, with H14-grade HEPA filters arranged on both sides of the plenum as the terminal filters. The airflow, after being filtered by the HEPA filters, is supplied into the plenum, then evenly discharged downward through the damping mesh beneath the plenum, and returns through the return air outlets installed at the lower part around the operating room to the air re-treated in the air handling unit. At the design air change rate, air is continuously filtered and treated by the system, ensuring that the cleanliness of the operating room remains above the design requirements.

In order to exhaust the disinfectant gases, anesthetic gases and unpleasant odors accumulated in the operating room, each operating room is equipped with an independent clean exhaust system, with the exhaust outlet located at the top of the operating room on the side of the patient's head.
